This voluntary, blended home visiting program is for pregnant women and families with children from birth to age 6. Our home visiting team consists of public health nurses, family resource workers (parenting peer support), and a social worker (settlement, employment /education attainment, finance, and housing support).
PrenatalPublic health nurses assess clients and provide information and support to promote a healthy pregnancy and baby. Clients who may require more support during or after pregnancy will be offered our home visiting program.
Postpartum Public health nurses contact new mothers to offer teaching and support, either by phone or during a home visit.
Families who may need ongoing support will be offered our home visiting program.
Families with Children 0-6 years A public health nurse will offer home visiting to families based on an in-depth assessment. Families are key participants in identifying goals that direct service provision, which includes linking to community services and supports.
Referral to Healthy Babies Healthy ChildrenTo access the Healthy Babies Healthy Children Program, clients can self-refer, or referrals may be made on their behalf by community service or health care providers.
